As of August 2024, I worked as a Product Designer Apprentice on a B2B healthcare management website. The website is used by over 600 staff to manage seniors' health records, financial information, appointment scheduling, and external volunteering records. Below project involved a new seamless platform for senior referral management in Active Ageing Centres. 

Information and Referral to Care Services

This feature offers a new platform within the website for Active Ageing Centres to serve as the main point of contact for social and health-related matters involving seniors and their caregivers.

Previous method

Centre staff currently rely on a shared spreadsheet (screenshot below) to manually track referral information and progress for seniors or their caregivers. This manual process often leads to inconsistent data, making it difficult to coordinate and share accurate data among team members.

Solution

Digitalising the existing shared spreadsheet by creating two separate sections within the website, they should each display: 

  • An overview of each senior's information and their referrals to care services

  • Detailed lists of information provided and referral records under individual senior profiles

Metrics

Previously, staff spent an estimated 30 minutes completing and coordinating senior details using the manual method. With this solution, we are targeting 100% adoption, aiming to significantly enhance operational efficiency and improve reporting capabilities.
